Under the Department are two divisions: Public Utilities, and Roads.
PUBLIC UTILITIES DIVISION
The Division provides services for water and sewerage.
ROADS DIVISION
The Division designs, constructs, and manages County roads, bridges, and associated structures. Service and maintenance garages are in Accident, Grantsville, and Oakland.

The Garrett County Sanitary District was incorporated in 1962 (Code Environment Article, secs. 9-601 through 9-699). In April 2003, the Board of County Commissioners became the governing body of the Sanitary District, which functioned through the County's Public Utilities Department (Code Environment Article, sec. 9-629). In October 2013, the Public Utilities Department was reformed as the Public Utilities Division of the Public Works Department.
The Roads Division originated as the Roads Department. It reformed as a division of the Public Works Department in October 2013.
